§1

(1) Vzor xxxxxxxxxxxxxx xxxx, xxxxx xxxxxxxx xxxxxxxx xxxxxxx xxxxx x xxxxx xxx s xxxxxxxxxxxxx xxxxx, xx xxxxxx x příloze č. 1 k xxxx vyhlášce.
